My first suggestion is go to your local library and borrow a copy of Budgeting for Infertility: Bringing Home Baby without Breaking the Bank. I wrote it, so I'm biased, but there are entire chapters on insurance coverage. You are absolutely right - it is on a case by case basic, depending on your state and your specific coverage. What is covered for your family might be (and probably is) different than what would be covered for the boss. So pull out the insurance manual and start digging.
