my wife's AMH is 18.54ng/ml what are the chances of concieving

AMH, or anti-mullerian

AMH, or anti-mullerian hormone is a substance that is produced by granulosa cells in ovarian follicles.

AMH levels do not change significantly throughout the menstrual cycle and decrease with age. Healthy women, below 38 years old, with normal follicular
status at day 3 of the menstrual cycle, have AMH levels of 2.0 – 6.8 ng/ml (14.28 – 48.55 pmol/L).

Anti mullerian hormone is one potential test of ovarian reserve. There are other tests that are currently used for evaluation of the remaining egg supply. None of the tests are perfect, and fertility specialists will often use a combination of tests to try to get a better estimate of the size of the remaining egg supply.

It is high, but please do remember that doctors do not treat numbers - they treat patients, so don't obsess over just one number is isolation. I would see a fertility specialist right away to determine your options.
